Lawrence M. Martin, M.D., P.C. is a solo private practice of psychiatry.  It has been determined that being a provider for multiple insurance companies at least triples the administrative work, substantially reduces payment, and adds significant constraints to providing optimum care to each patient.  To this end, it has been determined that WE DO NOT TAKE INSURANCE ASSIGNMENT INCLUDING MEDICARE AND TRI WEST.

Our policies are as follows:

1. Payment in full is due at the time of service.
2. Payment may be made in the form of cash, personal check or American Express, VISA, or MasterCard.  We do not accept Discover.
3. If a check is returned Insufficient Funds, a $25 charge will be incurred and we will ask future payments to be in another form.
4. We will request that an active credit card be kept on file for those charges incurred when the patient is not present i.e. telephone visits, document preparation/copying, or missed/late cancel appointments.
5. When payment for services is made we will provide a "Super Bill", documenting services and diagnosis which may be submitted to your insurance for out-of-network reimbursement for services.  Your insurance company can provide you with their schedule of payment for this.

3. Brief Medication Check 20 minutes......................................$118
4. Physician document review/preparation...............................$350/hr prorated
5. Record copying/sending (electronic)...................................$25
6. Record copying/sending (hard copy)...................................$35
7. Telephone visit (scheduled or urgent).................................$350/hr prorated
8. Late cancellations or no shows...........................................$100
9. Psychotherapy.................................................................$300/hr prorated
